Once the child is ready to attend the Malverne Afterschool Center, Inc. (MAC) program, the parent should notify the teacher in writing the date and frequency that the child will be attending MAC. The parent will also need to call the MAC office on the preferred start date.
It is the parent's responsibility to send new forms (Tuition Schedule/Fee Agreement (p. 1) and Promissory Note Agreement (p.10 or 11 for DSS-Parents) to the MAC Director or Business Manager regarding any changes to the days, times, and fees of child care There will be a charge of $25.00 administrative fee for any change resulting in a decreased tuition.
Timely payment of the correct fee amount is the sole responsibility of the parent, regardless of when or whether or not a bill is received from MAC for the service rendered.
The parent is responsible for picking up his/her child at the time designated on the enrollment agreement. In the event the parent cannot pick up, he/she must designate someone else to do so. If the child will be picked up late, MAC should be contacted before hand, regardless of pickup time. MAC staff will phone emergency contacts listed on the registration forms to pick up the child, if parent cannot be reached.
Please be respectful of hours of operation. Late fees are applied to late pick ups at $25.00 for every 15 minutes or any portion thereof. Late fees pay teachers for the extra time spent in MAC. Continued abuse of late pick-up may result in the child being discharged from MAC.
The child will not be released to anyone without authorization from parent and proper identification.
Parent should sign-in their child for the Morning MAC program and sign- out for the Afterschool MAC program every day and must record the drop- off or pick-up times, as required by the NYS OCFS. The daily fee for 6:30pm pick up will be charged each day the parent fail to complete the Sign In/Out sheet.
For a child enrolled on a daily basis, MAC must be notified no later that 5PM the day before the morning program and 12 NOON the same day for afternoon program on when their child shall be attending MAC, unless previously scheduled. Parents will be called if a child comes to MAC without prior notice. MAC will not be responsible for the whereabouts of any child who is not scheduled to be at MAC.
Parent must inform MAC of any special needs, problems, daily medication or know medical conditions that his/her child may have.
Parent must notify the MAC office ahead of time if their child will not be attending MAC on any scheduled day.
Parents of H.T. Herber Middle School students should notify MAC in advance if a child has detention after school or is going to be late because of an after school activity. MAC is not responsible for the child until he/she gets to the MAC program.
Registration fees is a one time, non-refundable charge. To save your child a spot in MAC, please pay the registration early.
Parents who wish to use the center on a monthly basis should pay the start up fees that consist of the September tuition, Registration, Fundraising and June/Last Month Deposit.
The start-up fees to be paid by parents wishing to use the center on a daily or as needed basis consist of 6 days (daily rate x 6 days) tuition, fundraising and registration fee.
A deposit due upon enrollment and shall be applied to the last month of the school year or the last month the child attended MAC that has not been paid.
If the child will be taken out of MAC earlier than June, the MAC lead teacher and the MAC office should be notified immediately. If the child attended a few days during his/her last month in MAC, the days attended will be paid based on the daily rate fees. If there is no unpaid balance, the June Deposit will be fully refunded.
IF JUNE DEPOSIT IS NOT PAID BY MARCH 10th, a $25.00 late payment fee will be charged until the full deposit is paid. There will be no MAC service for the child after May 10th, if the deposit remains unpaid.
If a parent owes a balance from prior years, full payment of balance, in addition to two (2) months deposit will be required, payable by CASH or money order.
The monthly/daily tuition payments are due no later than the 10th day of each month, otherwise, a $25.00 late fee shall be added to the bill. Non-payment will result in discharging the child from MAC and/or legal action will be taken to recover tuition payment with interest owed to the Malverne Afterschool Center, Inc.
An additional administrative fee of $30.00 (subject to increase based on actual bank fees charged to MAC) shall be billed for a bank-returned check. If this occurs again, only CASH or money order shall be accepted in future payments.

Operating Time Policy
When the school has a delayed opening, the MAC Morning Program will be closed.
If the Malverne School District opt for early dismissal, particularly, during inclement weather, under limited time and staff, MAC will hold any left behind MAC child until they get picked-up. However, please understand that MAC is also responsible for the safety of its staff. Arranging emergency pick-up for the child by an assigned relative would be much appreciated if the parent cannot pick up his/her child.
MAC IS CLOSED ON ALL SCHOOL BREAKS/HOLIDAYS.
MAC may opt to operate on early dismissal days depending on the number of students interested to participate in and for an additional fee.
Special Needs Policy
MAC will not refuse to admit a child to the program solely because the child has disability or has been diagnosed as having Human Immunodeficiency Virus (HIV), HIV- related illness or Acquired Immune Deficiency Syndrome (AIDS). Such child will be evaluated by the Director to determine whether the child could be accommodated in the program, if reasonable modifications are made to the premises and/or program.
Nothing contained in this paragraph shall be deemed to require the Malverne Afterschool Center to incur significant additional expenses to modify the premises and/or program to accommodate such a child.
MAC will not administer medication during its program hours.
MAC has the right to call the emergency numbers and arrange for someone to pick up the child who appears to be ill, show signs of contagious illness, or if MAC has to close due to an emergency situation, including heavy snowfalls.
First aid treatment can be given to a child by MAC staff for any minor injuries occuring while in MAC.
A child who does not behave appropriately will be spoken to by the staff in private. If undesirable behavior persists, the child will be given a 5-20 minute time-out. A child who continues to misbehave will be asked to leave the activity or stay away from other children with whome he/she is having difficulty. In the event that a child is a danger to him/herself and/or others and we cannot control the behavior, MAC reserves the right to discontinue service.
No parent is allowed to discipline a MAC child other than his/her own. All MAC students are encouraged to bring their concerns to their attending teacher.
Each family is required to participate in MAC fundraising activities and/or MAC program activities designed to keep tuition fee increases to a minimum. The amount of $60.00 candy sale fundraising (one of our main fundraising activity) fee will be charged at point of registration.
The MAC staff will be directing children to do their homework unless parents indicate that they do not want it to be done during MAC afternoon sessions. In accordance with New York State Office of Child and Family Services, no more than 45 minutes per session may be devoted to homework, in keeping with the developmental needs of the children. Staff will provide help, if asked, The staff is NOT responsible for correcting homework or checking that all homework is done. It is the parent's responsibility to review the homework with their child on a daily basis.
Late Pick-Up Policy
Late Pick-up is any minute after the designated pick-up time of 4:30pm or 6:30pm on any given day. A late pick-up fee of $25.00 will be charged for the first fifteen (15) minutes or a part thereof and another $25.00 for any additional 15 minutes of a part thereof. MAC reserves the right to discontinue care after three (3) late pick-ups.
DSS-Assisted Parent's Responsibilities
A parent may opt to apply for tuition assistance with the Nassau County Department of Social Services (DSS). Helpful information is available at the MAC office or you may call DSS Day Care directly at tel. # (516) 227-8519.
If a parent is approved by DSS, the parent should submit a copy of their authorization letter with their social worker's name, effectivity dates, and information on applicable parent fees that may apply. The parent should update MAC of their status of DSS eligibility on a weekly basis.
A parent awaiting approval from DSS may opt to enroll their child by initially paying the registration fee, fundraising and regular monthly tuition. If disapproved by DSS, the parent is responsible for payment of any additional unpaid regular tuition and deposits.
Any DSS-approved parent is responsible for paying the parent portion of the fee every week, when applicable. Failure to pay the parent fee will cause MAC to notify DSS and may pursue payment through legal or collection agencies.
DSS will only pay if parent signs in (for Morning Program) and/or signs out (for Afterschool Program) their child on the DSS attendance form. Parent will pay out of pocket if the child is not signed in/out on days attended.
General Fees Remittance Policy
Timely payment of the CORRECT FEE AMOUNT is the sole responsibility of the parent, regardless of when or whether or not a bill is received from MAC for the service rendered.
Payment is due on the 10th day of the month. If payment is not received by the end of the month, the child/children will be discharged, and/or legal action may be taken to recover tuition payment with interest owed to the Malverne Afterschool Center, Inc.
Payment can be made with personal check, money order, or cash. All check payments should be made payable to Malverne Afterschool Center, Inc. or MAC. All CASH payments should be brought directly to the MAC Main Office.
Payment can also be done via CHASE QUICK PAY, for parents who are Chase Bank online customers.
MAC DOES NOT ACCEPT CREDIT CARD PAYMENTS, NOR DO WE INITIATE/ARRANGE FOR AUTOMATIC REMITTANCES.
Sending payment with the child in his/her backpack is strongly discouraged. No school personnel nor MAC will be held responsible in the event the payment gets misrouted or lost. Applicable late payment fees of $25.00 will apply for the non-receipt of payment after due date.
